乳幼児健診における眼科的スクリーニング

Ophthalmological screening in the health check program for low-age children

　我々は，1982年4月から85年11月までの3年7カ月間に，乳幼児健診などの機会に眼科的異常を疑われ，眼科二次検診を受診した3歳未満の乳幼児298人の検診結果につき検討し次の結果を得た．

　受診者298人中二次検診で要精検とされた者は99人（33.2％）であった．このうち94人が精検を受け，異常ありとされた者は89人（29.9％）であった．この一連のスクリーニングでの異常者は96人（32.2％）で，斜視群62人，屈折異常群54人，その他の異常群22人であった．

　異常者の既往歴，全身状態などを調べたが，これらの中には周産期に異常のあった者が20人（20.1％）あった．また精神，運動発達遅滞を有するもの11人（11.5％）を含めなんらかの発達遅滞や身体的異常を合併する者が19人（19.8％）あり，特に斜視群に目立った．また外斜視の家族歴を有する者も多かった．

We evaluated 298 children under 3 years of age who were initially suspected as suffering from eye anomalies at an earlier, regular check-up program. More detailed examinations were indicated in 99 children (33.2%) out of the 298. Out of 94 children thus evaluated, 89 showed abnormal findings. Out of the 96 abnormal children in the total series, 62 manifested strabismus, 54 refractive errors and 22 other ophthalmological disorders.

In order to detect high-risk children, we evaluated the past history, psychological and systemic disorders, and the family history. Twenty (20.1%) of the 96 children had some disorders during the perinatal period. There were 19 children with mental or systemic disorders, including 11 mentally retarded ones as Down syndrome. These complications were more remarkable in children with strabismus. We also detected familial occurrence of exotropia.
