　77歳の男性の黄斑部に発生した大きい網膜色素上皮剥離にアルゴンレーザー光凝固を加えたところ，1カ月後に剥離部の色素上皮に裂孔が発生した．他眼にはすでに瘢痕化した老人性円板状黄斑変性症がみられた．

　網膜色素上皮剥離は4乳頭径大の大きいもので，その復位を目的として，辺縁に光凝固を行った．1カ月後に網膜色素上皮剥離の耳側と鼻側の辺縁で色素上皮が離断し，残った色素上皮は収縮して中央に集まり，縦走する大きい皺襞となった．両側の色素上皮の欠損部（裂孔）では脈絡膜血管がよく透見され，螢光造影で強い過螢光を示した．その後，網膜下に著しい結合織性の増殖組織を生じた．老人に生じる大きい網膜色素上皮剥離に自然に，または光凝固後に，このような色素上皮裂孔の発生することが注目されているが，わが国では本例のような症例は報告されていないので紹介した．

A 77-year-old male presented with detachment of the retinal pigment epithelium, 4 disc diameters in size, centered around the fovea in his left eye. The fellow eye manifested senile desciform macular degen-eration.

We treated the left eye by placing a row of argon laser photocoagulation along the margin of detached retinal pigment epithelium sparing the papillomacular bundle. Four weeks later, tears of retinal pigment epithelium developed at the temporal and nasal edge of the detachment. The remaining detached retinal pigment epithelium retracted centrally forming curled folds. Choroidal vessels were visible through the de-fect of the pigment epithelium. At the end of 2 years of follow up, marked fibrous proliferation developed in the subretinal space forming cicatricial mass.
