Abstract 文献概要

　Scleral buckling （SB）単独で治療された353眼のmassive proliferative vitreoreti—nopathy （PVR）（grade D）をrctrospectiveに分析し次の結果を得た。

（1）網膜の復位率はPVRが重症なほど有意に低下し（D−1-51.2％，D−2-43.3％，D−3-24.7％），全症例では42.5％であった。

（2）大裂孔，脈絡膜剥離，硝子体出1血は復位率を有意に低下させた。

（3）復位例の81.3％に指数弁以上の視力を得，復位例の術後視力は術前PVRの重症度と相関を示さなかった。

（4） SB単独療法はmassive PVRに対して一定の治療効果を有することが明らかとなり，PVRの治療上，基本的に重要な役割を果たすことが実証された。

From 1956 to 1966, 353 eyes with massive pro-liferative vitreoretinopathy (PVR, Grade D) were treated with scleral buckling alone without using silicone oil or hyaluronic acid. The retina was reattached for at least 6 months in 42.5% of eyes and the anatomic success rate decreased significant-ly with increasing severity of PVR. Eyes with larger breaks had a significantly lower reattachment rate compared to eyes with smaller breaks. Vitreous hemorrhage and choroidal detachment had signifi-cant association with anatomic failures. 81.3% of the anatomically successful cases achieved counting fingers or better postoperative vision.
