黄斑円孔にバックルをしない手術方法

A new management of retinal detachment with macular hole. Diathermy, drainage and air injection

　1981年5月から1983年4月までの2年間，出田眼科病院で手術を行った裂孔原性綱膜剥離379眼のうち，黄斑円孔のあるものは32眼（8％）であった。このうち後極部一帯の完全後部硝子体剥離のある18眼の黄斑円孔に対する処置は，黄斑部への眼底直視下のジアテルミー，網膜下液排出，硝子体内空気注入という術式を選択した。なお，後極部一帯の完企後部硝子体剥離がないか，あるいは不明の14眼には，さらに黄斑部へのシリコンスポンジ縫いつけによりバックルを行うという従来通りの術式を行った。また，周辺部裂孔を伴うものは32眼中9眼あったが，それらには周辺部裂孔に対する強膜内シリコン埋没術を併用した。

　その結果，バックルをしなかった18眼中17眼に，1回の手術で網膜の復位が得られた。他の眼は3回の手術を要したが，再剥離の原因は黄斑バックルをしなかったことではなく，周辺部に裂孔があったことと考えられた。また，バックルをした14眼中12眼が1回の手術で復位した。他の2眼も2回日の手術で復位した。

　以上より，完全後部硝子体剥離があり．網膜に対する硝子体の牽引がない黄斑円孔には，バックルは不要であることがわかった。これらには，このような適切な手術手技を用いれば，従来の方法よりはるかに短時間にかつ容易に治癒せしめることができる。

In the last two years between 1981 and 1983, 32 eyes of retinal detachment with macular hole were treated by us. They were divided into two groups. In group 1, posterior vitreous detachment (PVD) was present whereas in group 2 PVD was not present. 18 eyes were in group 1 and 14 eyes were in group 2. Nine eyes in both groups had peripheral retinal breaks combined with macular hole. In group 1, the macular hole was treated with macular diathermy under indirect ophthalmoscopic control, complete drainage of subretinal fluid and air injec-tion into the vitreous cavity.
