　21歳，女性にみられた黄斑部限局性網膜剥離の1例につきその臨床経過を報告した。黄斑剥離は，全層性と考えられる黄斑裂孔を伴い，また視神経乳頭にはoptic pitが認められた。網膜剥離部に光凝固を行つた所，網膜剥離の急速な消退，視力の著明な改善をえた。

　本症例の臨床経過より分層孔と全層孔の鑑別診断の問題点について考察した。光凝固の奏効機序としては，脈絡膜側への下液の排出路造設という考え方以外に，熱エネルギーによる浮腫液中の蛋白凝固にもとづく膠質浸透圧の低下という新しい仮説の可能性も検討した。

A 21-year-old female with pit macular syn-drome with a macular tear was presented. The macular tear appeared to be full thickness and the retinal detachment was limited in the macular area and was not connected with the pit in the optic disc.

Under the tentative diagnosis of the pit macular syndrome with a macular tear or rheg-matogeneous retinal detachment, the patient was successfully treated by argon laser photo-coagulation. Four months after the photocoa gulation, there was recurrence of the detach-ment with the characteristic features of the pit macular syndrome with a lamellar tear.
