緒　言

　緑内障における，瀘過手術後の合併症は，その修復が非常に困難なものが多く，今日に至るまでの緑内障手術術式の変遷は，合併症との戦いといつても過言ではない。近年，従来の術式に比べ，比較的合併症の少ないtrabeculectomy，trabecul-otomy，sinusotomy等の術式が考按され，特にtrabeculectomyは，その適応範囲の広さから，好んで用いられている術式である。しかしながら，trabeculectomy自体が，瀘過手術である以上，併発率の低下はあつても，合併症の重篤さは，旧来と変わるものではない。とりわけ，術後前房再生遅延は，眼科医が必ずや遭遇する難題の一つであろう。

　今回，われわれは狭隅角性緑内障眼にtrabecu-lectomyを施行した後，胞状脈絡膜剥離を伴い前房再生遅延をきたした症例を経験し，これを手術的に治癒せしめたので，その発生機序および治療法等につき若干の考察を加えてみた。

We experienced a case of long-standing hy-potony and flat anterior chamber with bullous choroidal detachment following trabeculectomy for narrow-angle glaucoma.

By drainage of subchoroidal fluid and by injec-tion of air into the anterior chamber, the cham-ber was successfully restored and the visual acuity was improved from the presurgical level of 0.2 to 0.6.

The main factors of hypotony and flat cham-ber are considered to be (1) cyclodialysis follow-ing surgical manipulation, (2) hyposecretion of aqueous fluid, (3) aqueous flow into the supra-choroidal space, and (4) bullous choroidal de-tachment.
