近在感輻輳過剰による眼精疲労とγ−系抑制剤による治療効果

Increase in proximal convergence as a cause of asthenopia and its treatment with pyrodi fenium bromide

緒　言

　眼精疲労（Asthenopia, Mackenzie，1843^1））は，文明の発達に伴つて増加しつつある眼病であり，その治療も難しいものが多い。眼精疲労の原因は，眼自身または眼以外の体にあるもの（内因性），環境や不適正な眼鏡によるもの（外因性）など複雑ある。内因性と思われるものは，原因によつて更に幾つかに分類されている。一般に1）調節性眼精疲労（老視・遠視・乱視など，Jackson，1885^2）），2）筋性眼精疲労（斜位など，vonGraefe,1862^3）），3）症候性眼精疲労（結膜炎など），4）不等像性眼精疲労（アニサイコニア，Ames,1932^4））などに分類され，これらのいずれの原因も発見できないものを，5）神経性眼精疲労として一括する人が多い。私共はこの分類によるいわゆる神経性眼精疲労に所属する症例の中に，近在感幅榛（Proximal convergence）が異常に大きいものが多いこと，およびそういう症例には臭化プリフィニウム（パドリン®）を投与すると著効を奏するものがあることを見出した。

Asthenopia is a disease known for a century, but its incidence is increasing recently accom panied by the rapid development of modern civili zation. In many cases of this condition, doctors fail to detect either ocular factors such as phoria, refractive errors, presbyopia and conjunctivitis, or probable exogenous factors causing this condi-tion. We examined the state of convergence in 61 patients and 90 normal controls. We found that the proximal convergence tended to be greater in the former than in the latter.
