緒　言

　頭蓋内に同時に2個以上の動脈瘤が存在する多発性脳動脈瘤は脳動脈瘤の症例全体の9〜21％^1）に存在するといわれ，それほど珍しいものではない。しかしながら内頸動脈海綿静脈洞部の両側性の動脈瘤はこれまで海外では22例，本邦では3例の報告しかなく，きわめて稀な疾患である^2）。このたび著者らは21歳男性で左の外転神経麻痺で始まり脳血管撮影の結果，両側の内頸動脈海綿静脈洞部に巨大な動脈瘤が発見された症例を経験したので報告する。なお著者らの症例は両側性内頸動脈海綿静脈洞部の動脈瘤のこれまでの内外の報告例のなかでは最年少である。

A 21-year-old male sought medical advice on account of diplopia which started 2 months before and which had been gradually progressing. Neurological examinations disclosed a moderate paresis of the left 6th cranial nerve. Routine laboratory examinations were inconspicuous. Skull X-ray disclosed no abnormal findings. Carotid angiography demonstrated the bilateral presence of huge aneurysms of internal carotid arteries within the cavernous sinus, measuring 30×21×23mm on the left and 21×20×28mmon the right side. The aneurysms could not be treated by surgical means because of the bilateral involvement.
