緒　言

　白内障の手術中におこる硝子体脱出を予防し，またはこれを救済する方法については多数の報告がある。従来一般に行なわれている防止対策は，十分に瞬目・球後麻酔を効かせる，眼球マッサージをする，MannitolやDiamoxなどを投与する，などの方法で眼圧・硝子体圧をコントロールすること，またはFlieringa輪を使つて眼内圧の上昇を防ぐことなどであり，救済対策としてはanterior vitrectomyがある。

　三井は1966年に，硝子体脱出は手術前または手術中に予測できることが多いことを述べ，こういう場合には大きな周辺虹彩切除孔をつくつて，そこから水晶体を摘出すれば，硝子体脱出をかなり防げると共に，たとえ硝子体が周辺虹彩切除孔から脱出しても，良い視力と，円形の瞳孔とを保つことができる事を発表した^1）。

Mitsui published a cataract extraction through peripheral iridectomy as a preventiveand a relief method of the vitreous loss in 1966. In this study 252 cases of senile cataract were reviewed about the postoperative visual acuity and the complication to evaluate the results of this method.

(1) The 11 cases in which the vitreous loss happened through the pupil showed the poor postoperative visual acuity (the average 0.61) and resulted in high percentage of postoperative complications (18%).
