Retinal arterial aneurysms 8症例の臨床像と光凝固治療

Clinical Findings and Photocoagulation in Eight Cases of Retinal Arterial Aneurysms

緒　言

　網膜動脈に，まれに孤立性の比較的大きな血管瘤を認めることがある。この動脈の血管瘤より限局性の出血や浮腫を起こし，輪状網膜症様の所見や，網膜中心静脈枝閉塞症類似の変化をきたす。最近この網膜動脈の血管瘤により起こる臨床像を，一つのClinical entityとして報告されるようになつた^1〜7）。Robertson^1）は，この網膜動脈の血管瘤に対し，Macroaneurysmなる名称を与え，Leber病，Coats病，Hippel病などと異なり，その臨床像を，1）主幹動脈系（第3分枝以内）に血管瘤を認める。2）限局性の網膜出血と浸出斑を伴う。3）高血圧，動脈硬化症のある高齢者に多い。4）特別な治療なく自然寛解の傾向があると述べた。

　しかし，Cleary^4）は，黄斑部に浮腫のおよぶ症例は，血管腫自体は自然治癒しても，浮腫によつて黄斑部の器質的障害を起こし，視力障害の原因となると述べており，すべての例で，Robertsonのいうように，全く治療せずに放置しておいてよいか，疑問を投げかけている．この血管瘤自体に光凝固を行うことにより，血管瘤よりの透過性をおさえ，浮腫の吸収の促進をはかることにより，術後の視力の維持に好結果をもたらし，また，凝固治療による重篤な障害も認められなかつた。そこで網膜動脈の血管瘤に対する光凝固治療の適応を主体に述べてみたい。

We observed eight cases of retinal arterial aneurysm in elderly subjects of more than fifty years of age. Unilateral involvement was the rule and the aneurysm was always solitary. Visual impairment was either due to serous detachment of the macula, extension of circi-nate lesion onto the macula or superficial or sub-retinal hemorrhage from the aneurysm. Fluo-rescein angiography revealed extravasation of dye from the wall of the aneurysm in all the cases.

We treated these eight cases by directly ap-plying xenon or argon laser to the aneurysm.
