輪状網膜症の光凝固療法

Photocoagulation in Circinate Retinopathy

I．緒言

　輪状網膜症Circinate retinopathyには，黄斑部をとり囲んで生じる定型的なもののほかに，特に特定しない眼底部位に硬性白斑のやや不規則な輪状の集合として起こることがあり，糖尿病性網膜症，高血圧性網膜症，Coats氏病，Leber氏病（miliary angiomatosis ofthe retina）の部分症状として，しばしばみられる。これら続発性の輪状網膜症では，通例として輪状白斑の内部に毛細血管瘤で代表される網膜血管病変が存在することが眼底検査ならびに螢光眼底造影で確認され，しかも光疑固によりこれら異常血管を破壊することにより輪状白斑の消失をみることを比較的長期間観察できた4例について経験したので以下に報告する。

The clinical course of four instances of circi-nate retinal lesions treated by photocoagulation is described. Three cases were diagnosed as miliary angiomatosis of the retina (Leber) and the other as diabetic retinopathy. The circinate lesions were characterized by the presence of one giant aneurysm in its center accompanied by capillary microaneurysms, retinal telangiec-tasis and, in one case, localized dilation of reti-nal arteriole (Fig.1 in the text).
