Dyeレーザーを用いた糖尿病性黄斑症に対する格子状網膜光凝固の検討

Treatment of diabetic maculopathy using dye laser as grid pattern photocoagulation

　黄斑部にびまん性浮腫のある糖尿病性黄斑症18例25眼に対して，dyeレーザーによる格子状網膜光凝固を施行した。その結果，術後2段階以上の視力の改善が7眼（28％）にみられ，黄斑浮腫の改善は16眼（64％）であった。12例での静的視野（中心30°）による計測では有意な術後の感度低下を認めなかった。総合判定において，有効群は16眼，64％であり，不変群は7眼，28％，悪化群は2眼であった。dyeレーザーの波長による相違は認めなかった。

　dyeレーザーによる格子状網膜光凝固は有効であると考えられた。

We applied dye laser photocoagulation in grid pattern in treating 25 eyes, 18 patients, with diffuse macular edema due to diabetes. Improvement in visual acuity resulted in 7 eyes (28 %) . Macular edema diminished in 16 eyes (64 %). There was no significant difference in retinal sensitivity in cen-tral visual field before and after treatment. As anoverall evaluation, the treatment induced improve-ment in 16 eyes (64 %), no change in 7 (28 %) and deterioration in 2 (8 %). There was no difference in the effect between two wavelengths of 590 and 610 nm.

The finding thus shows that the present technique is of value in the treatment of diabetic macular edema.
