先天盲開眼手術後の視知覚獲得過程の観察

Observation on the Postoperative Formation Process of Visual Perception in the Early Blind

緒　言

　生来性の盲（congenital blindness）^1）あるいは幼児期に失明した盲（early blindness）^1）などのいわゆる"先天盲"が成人してから開眼手術を受け，開眼した場合の視知覚獲得に関しては，心理学的な見地から，これまでに多くの研究がなされている^1〜5）。それらによれば，その獲得過程は，その患者の失明時期，失明期間，失明期間中の残存視覚^2），開眼手術を受けた年齢^6），手術後の訓練^7），などに依存しており，開眼したからといつてすぐに視覚の世界に入り込める訳ではない。その上，ある種の視覚機能は，放置しておけば獲得されないままになりうる可能性が注目され，手術後の訓練の重要性が近年強調されている^7,8）。

　今回，著者らは，幼児期に麻疹後の角膜症で両眼失明した28歳の男子に，角膜移植を行ない，手術前後の眼機能の変化と，術後の視知覚獲得の過程を，およそ1年間観察したので報告する。

Studies were made on the formation process of visual perception in the early blind who regained sight by penetrating keratoplasty after 23 years of blindness. He lost his eyesight due to leukoma after measles when he was 5 years of age, and underwent successful graft surgery at the age of 28 years.

The following conclusions were obtained through observation on his visual formation process.
