Multifocal Posterior Pigment Epitheliopathy, Clinical Features and Treatment with Photocoagulation

緒　言

　眼底後極部に黄白色の滲出斑が散在性に多発し，網膜剥離を伴う疾患が最近注目されている。この疾患は，従来，原因不明の播種性網脈絡膜炎と診断されていたと思われるが，螢光眼底検査法や，網膜色素上皮を中心とした眼底病変についての知見によつて，最近一つの独立した疾患であるとの見方が強まり，今までに，網膜剥離を伴つた中心性脈絡網膜炎（浦山^1）），周辺性網膜剥離を呈する特異な網脈絡膜病変（三村^2,3）），特異な続発性網膜剥離（塚原^4），5）），異型中心性脈絡網膜症（吉岡^6,7）），bullous retinal retinal detachment，an unusual manifestation of idiopathic centralserous choroidopathy （Gass^8））などの病名で報告され，さらに同じ内容の症例は，uvealeffusionとして清水^9），三村^10）の報告もある。これらの論文をよくしらべると，どの報告も同一疾患を取扱つていると思われ，さらに，われわれは最近これらの報告例と同じ疾患とみなしうる症例の18例35眼を経験し，その観察結果から，この疾患は，独立した一つのclinical entityをもつ疾患であると考えるに到つた。その臨床所見を検討して，本疾患に多巣性後極部網膜色素上皮症multifocal posterior pigment epitheliopathyと命名した。

Several reports related to the disease of secondary serous retinal detachment with multiple retinal exudations located in the poste-rior pole of the eye recently appeared in American journal (by Gass) and in Japanese journals (by Urayama, Mimura, Tsukahara and Yoshioka). The disease may be occasional-ly misdiagnosed as focal choroiditis or rheg-matogenous retinal detachment, and may be frequently confused to central serous retinopa-thy, and uveal effusion of Schepens.

Thirty-five eyes of 18 cases (15 men and 3 women) were referred to the authors, and 13 eyes of 7 cases were treated with photocoagu-lation.
