多発性後極部網膜色素上皮症—最近の経験症例

Recent cases of multifocal posterior pigment epith-eliopathy

　最近6年間に経験した多発性後極部網膜色素上皮症の13例，19眼の症例から本症についての以前の筆者らの見解を一部修正した。治療には光凝固のみが有効であり，一度治癒すると再発をみないと前に述べたが，その後再発があることがわかった。また原田病初期と似た症例や，中心性漿液性網脈絡膜症との中間型といえる症例があった。さらにステロイド剤の全身投与が本症発症の誘因となったり，中心性漿液性網脈絡膜症様の前駆症状から急性増悪の誘因になっていた症例を経験した。その誘発機序を考按した。本症と中心性網膜炎とは同じ類型（Spectrum）に属する疾患であるが，明確な独立した疾患概念をもつ1病型であることを確認した。

We reviewed the clinical features of multifocal posterior pigment epitheliopathy (MPPE) seen dur-ing the foregoing 6 years. The series comprised 19 eyes, 13 patients. While photocoagulation was effective in inducing resolution of the disease, therewere cases of late recurrence in spite of the treat-ment. There were cases simulating Harada's dis-ease or central serous retinopathy. Systemic cor-ticosteroid triggered onset or exacerbation of the disease in some. These findings were in accordance with our previous description of the disease in 1977 with slight modifications.
