ネオシネジンと白内障手術

Neosyrin and Cataract operation

1．緒言

　著者等は先に臨眼誌上に「ネオシネジン」が眼科領域に於て瞳孔散大作用が速かで眼圧亢進の恐れなく，且つ散瞳時間も長くない上に1％液でも充分に散瞳効果があり，その場合0.5％のピロカルピン点眼により散瞳を阻止出来る事を知り診断的散瞳薬として理想的である事を発表した。その当時本剤の血管収縮作用も仲々強力である事を知つたのである。一方最近の老人白内障手術の傾向として所謂水晶体円形瞳孔手術が多く用いられる様になり，既に井上，桑原，大橋氏等の白内障手術に10％ネォシネジン応用の記載がある。更に近来カストロビェホー式の全摘鑷子，ベル式吸引器等々優秀且つ使用に便な器械の出現により更にこの傾向は強まろうとしている。然してこの手術に際し問題となるのは瞳孔径である。然し折角散瞳しても充血が強くなつたり，眼圧を上昇させたりしたのでは，手術に際し忌むべき硝子体脱出，虹彩脱出，前房出血等を来す恐れが多くなつて来る。此等の症状を避けるため従来は「アドレナリン」の結膜下注射，又は前日乃至数時間前の「ホマトロピン」点眼が用いられていたが色々と不便の点があつたのである。そこで著者等は今回「ネオシネジシ」の散瞳を白内障円形瞳孔手術に応用した結果，見るべき効果を得たので報告する次第である。

We have previously reported Neosynesin as a Superior drug for diagnostic mydriasis, Cata-ract operation was carried out by making use of its effect to dilate the pupils, to cause the vasoconstriction, and to decrease the ocular tension. The results of the operation were as follows:

A drop of 50% Solution was administered 30~60 min. before the operation.

Following the cocaine anesthesia, the pupillary diameter after the opening of anterior cham- ber Was measured under the same degree of illumination with vernier Calliper (1/10 mm. measurable). Pupillary diameter which had been less than 6mm during 30~60 min.
