緒言

　原発閉塞隅角緑内障の治療は外科的なそれが第一とされているが，その術式の選択，就中虹彩切除術，濾過手術のいずれをとるかの選択にさいして，隅角検査所見およびトノグラフィーによる房水流出率が重要な資料となることはいうまでもない。

　原発閉塞隅角緑内障の眼圧上昇機転は，虹彩根部による隅角線維柱帯の閉鎖であることは周知の事実であるが，この隅角閉鎖には二種類あるとされている。一つは虹彩根部が単に隅角線維柱帯に接しているだけで器質的癒着のない可逆的なもので，いわゆるappositional closureと呼ばれているものである。もう一つは両者の間に不可逆的な器質的変化，すなわち周辺虹彩前癒着（以下PASと略す）が存在するものである。これらの二者の鑑別は圧迫隅角検査を行なうことにより可能である。

A total of 120 eyes (80 cases) with chronic angle closure glaucoma were examined by me-ans of indentation gonioscopy, tonography and tonometry before and after surgery, with a vi-ew to obtain informations as to the choice of surgical method in this type of glaucoma.

1. Whenever the extent of peripheral anteri-or synechia (PAS) involved less than 80% of the circumference, there was a strong likeliho-od that the tension can be controlled by iridec-tomy followed by miotic treatment. When the extent of the PAS was less than 50%, the ten-sion could be kept within normal range by iridectomy alone.
