眼窩に発生した骨の好酸球性肉芽腫の1症例

A Case Report of Eosinophilic Glanuloma

緒言

　Eosinophilic granuloma of bone （骨の好酸球性肉芽腫）は，細網内皮細胞の増殖を基盤とし，多数の好酸球の浸潤を伴つた特徴のある組織像を示す比較的まれな疾患であり，1940年Lich—tensteinら^1），Otaniら^2）により命名されたものである。さらにその後Lichtenstein^3）（1953）はeosinophilic granuloma of bone, Hand-Schüller-Christian病，Letterer-Siwe病の3者は病理学的には細網内皮系の疾患という点で一致しており，これらは同一基礎疾患の異なつた臨床病型であると述べ，一括してHistiocytosis Xと総称することを提唱した。

　私達は，左側の眼瞼腫脹を主訴とした患者のレントゲン像より，eosinophilic granulomaの存在を疑つたが，病理組織学的にもそれが証明され，腫瘤摘出後にはステロイド療法を行なつて好結果を得た1症例を経験したので報告する。

A case of eosinophilic granuloma of the orbit was reported. The patient, an 8-year-old boy, was admitted with swelling of the left upper eyelid, but no spontaneous pain. Ophthalmolo-gical examinations found a slight exophthalmos of the left eye. The visual acuity, the extra-ocular movements and the fundi were normal. On admission, liver and spleen were slightly enlarged. The E. S. R. was 19 mm/hour and 42 mm/2 hours. The peripheral blood analysis was normal.

Roentgenogram of the bony orbit showed a well-defined osteolytic lesion on the left orbital roof.
