小児の眼窩骨好酸球性肉芽腫

Eosinophilic granuloma of orbital bone in children

　小児の眼窩骨好酸球性肉芽腫と思われる3症例を経験した。いずれも有痛性の上眼瞼腫脹で発症し画像検査で眼窩骨の融解を伴った軟部組織腫瘤がみられた。悪性腫瘍が疑われたが症状は改善傾向を示した。超音波検査では点状，顆粒状の軟部組織腫瘤陰影とそれを取り囲むように不規則な高反射を示す眼窩骨陰影が描出された。生検では黄褐色の柔らかい腫瘤組織が得られ，病理組織検査にて好酸球の浸潤を伴った異型組織球の増殖がみられた。その後，特別な加療もせず軟部組織腫瘤の完全吸収と眼窩骨の修復がみられた。3症例とも現在まで再発および他の全身症状を認めていない。

We observed eosinaphilic granuloma of orbital bone in 3 children. It was detected by painful lid swelling as the initial sign. Imaging studies showed a soft tissue density mass with bone defect simulating malignant orbital tumor. Echography showed a soft tissue density mass containing multiple dots and blots surrounded by irregular orbital bone image of high reflectivity. Biopsy showed yellowish brown friable tissue composed of histiocytic proliferation infiltrated by eosinophils. Biopsy was followed by spontaneous and complete resorption of the mass with reconstruction of the orbital bone. There has been no recurrence or systemic involvements so far.
