網膜細動脈のBright Plaqueについて

Bright Plaque in the Retinal Arteriole

I．緒言

　1958年Hollenhorst^13）が初めて頸動脈閉塞性疾患を有する例の網膜細動脈分岐部にしばしば認められるbrightorange plaqueが栓子によるものであろうことについて注意を喚起し，同年Witmer and Schmid^37）も高血圧で治療中脳乏血症状のため長期間治療されていた54歳の1例で，急に左眼窩部に疼痛を自覚したが，視力，視野に異常なかつた。眼底には，光輝ある短形濃黄色物が上耳側動脈の第1分岐部にみられ，混濁物は動脈管腔をみたしていたが，動脈閉塞症状はなかつた。そして，このものは眼底血圧測定時，拡張期圧に一致すると，血柱は虚血状態となり，乳頭面上まで移動し，収縮期血圧に近づくと，再びもとの分岐部にもどるのをみ，氏はその色，形からこれをコレステリン結晶と考えた。

A description is presented on sixteen cases in whom plaques were ophthalmoscopically seen in the retinal arterioles.

Characteristic features were as follows :

1. The occurrence of plaques became more frequent with increasing age with a maximum in the seventh decade. 14 males were affected against only 2 females. The occurrence was unilateral in all the cases. In 6 cases, the condition was present without any subjective symptoms and was detected on routine fundus examinations.

2. The plaque appeared white in 3 instances and orange yellow in 13 instances.

3. The condition was associated with cardio-vascular diseases (62.5%).
