Bright Plaquesを伴つた網膜中心動脈枝閉塞症

A Case of Retinal Embolism with Bright Plaques in Retinal Artery.

I．緒言

　網膜中心動脈および中心動脈枝閉塞症についての報告は，数多くなされている。この原因として，血管壁の病変説，血栓説，血管攣縮説，血液変性説などがあげられ，栓子によるとする報告は少ない。最近外国文献に於て，血管学殊に頸動脈のThrombo-endarterectomyの発達と共に，網膜中心動脈閉塞症様の症状を示す者の眼底に，栓子を認めたという報告が多くみられるようになつた。しかし目本に於ては，栓子によるとする報告は非常に少く，更に栓子を眼底に認めたという報告はごくわずかである。古くは稲富^1）により，最近は浅野^2）が脳卒中の患者の眼底に栓子を認めたと報告している。後者に於て眼症状はのべられていない。前者の報告に於ては，栓子を真の栓子とせず，動脈壁の局所的変化により起つてきたものと説明している。私は最近，眼底に輝いた斑点（Bright Plaques）を伴つた網膜中心動脈枝閉塞症の1例を経験したので報告する。

A description is given over a case of emb-olism of central retinal artery in a 66-year-old male. As a particular finding of this case bright plaques were present in retinal arterial branches and were interpreted as cholesterol crystals which probably migrated after obst-ructing the central retinal artery. Fluorescein fundus photography performed three weeks after onset of visual failure revealed unimp-aired blood flow at the sites of bright plaques. Through intensive treatment for 12 days the visual acuity improved from 0.4 to 1.0 with enlargement of the visual field.
