球後視神経炎を乳頭黄斑線維束炎と改称することを提案する

A Proposal About the Terminology of Neuritis Optica Retrobulbaris Changing its Term to Fasciculitis Papillomacularis.

　そもそも，病名はそれが固有名詞的であれ，症候的又は病理組織学的であれ，理屈にあつたものでなければならないのは論をまたないが，又国際的に通用しなければ意味をなさない。その上出来れば，その病名を見て誰にでも直に理解されたものが望ましい。理論によつて病名が変つて来た例の1,2を挙げれば，蛋白尿性網膜炎Retini—tis albuminuricaが腎炎性網膜炎Retinitis nephriticaとなり更に腎炎性網膜症Retinopathia nephriticaになつた例であり，又色素性網膜炎Retinitis pigmen—tosaが網膜色素変性Degeneratio pigmentosa retinaeに変つた例であろう。国際的に通用しない例としてはHemeralopia, Nyctalopia, Iridectomia basalisが代表として挙げられる。これから述べる球後視神経炎は欧米でも理論的に異論があり，又我が国の多くの人々の考えている定義と欧米の多くの人々の考えている定義とが可成り異なる様に思われ，このままでは国際的に通用しない恐れがあると私は心配しているので標題のことを提案したいと思う。

　球後視神経炎はA．v．Graefe （1866）がretrobul—bäre Neuritisと唱え出してから，一般に用いられる様になつた（小口^1），桑島^1），弓削^2）等による）。

Retrobulbar neuritis is not exactly defined even in Europe and America at present, but many ophthalmologists have accepted the concept by Rönne and Walsh indicating selec-tive involvement of the papillomacular bundle at any level in the optic pathways from the eyeball to the geniculate body. They are considering retrobulbar neuritis as identical to "axial optic neuritis" by Wilbrand and Saeger. In Japan, however, the terms are not considered as the synonym interpreting stric-tly the term named "retrobulbar".
