要約　目的：淋菌性角結膜炎に関節炎症状を伴った症例の報告。症例と所見：27歳男性が両眼の眼脂と充血，多関節痛で受診した。両眼瞼および球結膜は腫脹し，クリーム状の膿性眼脂が多量に付着，右眼角膜潰瘍があった。左肩と左足背，背部胸椎棘突起に圧痛があり，左足背には腫脹があった。臨床症状から淋菌感染症として，抗菌薬点滴静注および頻回点眼を開始し，角結膜炎は軽快した。治療中，眼脂PCRにて淋菌DNAが検出された。その後も多関節炎と炎症反応は遷延したため，反応性関節炎としてステロイド薬と抗リウマチ薬の内服加療を開始し，緩徐に改善した。結論：淋菌感染症ではさまざまな全身症状に注意し，適切な治療を選択する必要がある。

Abstract.　Purpose：To report a case of gonococcal keratoconjunctivitis associated with polyarthropathy. Case：A 27-year-old male presented with discharge and hyperemia in both eyes since 2 weeks before. He had pains in multiple joints. He had been diagnosed with urethritis 10 days before. Findings：Both eyes were swollen and showed creamy discharge. He complained of tenderness in the left shoulder, left dorsum pedis, and thoracic spine. He was treated with intravenous ceftriaxone and topical levofloxacin and cefmenoxine. Keraconjunctivitis subsided in 5 days. Arthritis persisted after switching to minocycline. Diagnostic imaging showed findings of reactive arthritis. Systemic corticosteroid and antirheumatic drugs resulted in gradual improvements 3 months after onset. Conclusion：This case illustrates that infection by Neisseria gonorrhoeae may induce a variety of systemic lesions, including arthritis.
