ステロイド点眼液により結核性ぶどう膜炎を治療した1例

A case of tuberculous uveitis treated by topical corticosteroid

要約　背景：ジフルプレドナート（difluprednate）は消炎効果のあるステロイド剤で，点眼液として用いた場合，眼内移行性が高いとされている。目的：網膜血管炎を伴う結核性ぶどう膜炎をステロイド点眼で治療した症例の報告。症例：58歳男性が2か月前からの右眼霧視で受診した。15年前に甲状腺癌の手術を受け，以後2回の転移があった。所見：矯正視力は右0.01，左1.2で，右眼には，前房に炎症所見，硝子体混濁，びまん性の網膜出血，白線化を伴う静脈周囲炎があった。ツベルクリン反応が強陽性，クオンティフェロンTB-2G陽性であり，胸部X線検査では結核を示す所見はなかった。結核性ぶどう膜炎を疑い，抗結核薬4剤併用による治療を開始した。2か月後に右眼の病的所見が軽快したが，黄斑浮腫が残っていた。1日4回の右眼への0.05％ジフルプレドナート点眼を開始した。3か月後に病的所見は改善し，右眼視力は0.2になった。以後8か月後の現在まで，炎症の再発はない。結論：網膜血管炎に併発した黄斑浮腫が，ジフルプレドナート点眼で軽快した。

Abstract.　Background：Difluprednate ophthalmic solution（Durezol^®）is claimed to be an anti-inflammatory agent with high intraocular penetration. Purpose：To report a case of presumed tuberculous uveitis with retinal vasculitis treated with diflupredonate ophthalmic solution. Case：A 58-year-old male presented with blurring of his right eye since 2 months before. He had received surgery for thyroid carcinoma 15 years before. Findings：Corrected visual acuity was 0.01 right and 1.2 left. The right eye showed signs of inflammation in the anterior chamber, vitreous opacity, multiple retinal hemorrhages, and perivasculitis. He was positive to tuberculin skin test and QuantiFERON TB-2G. He was diagnosed with presumed tuberculous uveitis and started receiving peroral antituberculous medications. Ocular signs of inflammation subsided 2 months later leaving macular edema. Instillation of 0.05％ difluprednate ophthalmic solution was followed by improvement of macular edema after 3 months with visual acuity of 0.2. There has been no recurrence for 8 months until present. Conclusion：Instillation of difluprednate ophthalmic solution was followed by improvement of macular edema in a case of retinal vasculitis.
