Abstract. Background:It has been customary to have the patient in prone position for 4 days after macular hole surgery. Purpose:To report the outcome of macular hole surgery after prone positioning for 4 days in one group and for 6 hours only in the other. Cases and Method:This retrospective study was made on 23 eyes of 23 patients who received vitrectomy and air-fluid exchange for idiopathic macular hole with diameter of less than 400μm. Eleven patients remained in prone position for 4 days and 12 patients for 6 hours after surgery. Cases were followed up for 3 months or longer. Results:Macular hole closed in all the 23 eyes after initial surgery. There was no difference in the postoperative course of visual acuity in both groups. Intraoperative retinal tear occurred in one eye in 4-day group and in 2 eyes in 6-hour group. Conclusion:Prone positioning for 6 hours after surgery for small macular hole may be sufficient to induce closure of the hole.
