要約　目的：ストレートチップを装着して先端が左右に動くトーショナル水晶体乳化吸引術の有効性と問題点の報告。対象と方法：核硬度が2～3.5に相当する白内障84眼に手術を行った。33眼にはストレートチップを装着し，核の分割と破砕を試みた。51眼ではストレートチップとKelmanチップで，トーショナルと縦振動の組合わせによる総エネルギー量と超音波発振時間を測定した。結果：核硬度3まではストレートチップによる破砕吸引が可能であった。ストレートチップのトーショナルモードに超音波による核破砕を加えると，破砕力はKelmanチップより劣るが，従来の縦振動より優れていた。結論：ストレートチップによるトーショナル水晶体乳化吸引術白内障は可能かつ有効であった。

Abstract.　Purpose：To report the outcome of longitudinal and torsional phacoemulsification with straight and angled tips. Cases and Method：Cataract surgery was performed on 84 eyes with density grade from 2 to 3.5 after Emery-Little system. Straight tip was used to divide and fracture the nucleus in 33 eyes. Straight and Kelman tips were used in 51 eyes to measure the total energy and duration of ultrasound application using torsional and longitudinal vibration. Results：The lens nucleus could be divided and aspirated by straight tip in eyes with density grade of 3 or less. Straight tip was less efficient than Kelman tip but was more efficient than conventional longitudinal vibration when added by ultrasound nuclear division. Conclusion：Torsional phacoemulsification-aspiration is possible and efficient using straight torsional tips.
