短期間に寛解と増悪をみた網膜色素上皮剝離の1例

A case of pigment epithelium detachment with rapid exacerbation and remission

要約　目的：漢方薬の内服と鍼治療で短期間に消失した網膜色素上皮剝離の1例の報告。症例：アレルギー性結膜炎の既往がある54歳女性が2年前からの右眼の変視症で受診した。所見：右眼黄斑部に網膜色素上皮剝離があった。漢方薬の内服で2年7か月後に軽快し，内服を中止した。その5か月後に再発し，病巣が瓢箪形になった。さらに7か月後に左眼黄斑部に網膜色素上皮剝離が発症した。鍼治療と漢方薬内服で，5か月後に左眼は治癒し，その2か月後に右眼の漿液性網膜剝離が消失したが，網膜色素上皮剝離は増悪と再発を繰り返している。結論：漢方薬と鍼治療が網膜色素上皮剝離の軽快に関与した可能性がある。

Abstract.　Purpose：To report a case of retinal pigment epithelium（RPE）detachment that showed rapid exacerbation and remission after treatment with systemic Chinese medicine and acupuncture. Case：A 54-year-old female presented with metamorphopsia in the right eye since 2 years before. She had a history of allergic conjunctivitis. Findings：Her right eye showed RPE detachment in the macula. It became less conspicuous after 31 months of treatment with peroral Chinese medicine. It recurred 5 months after discontinuation of treatment to assume a sandglass shape. RPE detachment occurred in the left macula another 7 months later. Treatment with peroral Chinese medicine and acupuncture was followed by cure in the left eye 5 months later and by remission in the right another 2 months later. RPE detachment has been recurring with remissions during the ensuing 6 months. Conclusion：Treatment with peroral Chinese medicine and acupuncture may have induced remission of RPE detachment.
