サルコイドーシスに合併した網膜色素上皮剝離の経過

Clinical course of retinal pigment epithelial detachment in a case of ocular sarcoidosis

要約　目的：サルコイドーシスによるぶどう膜炎の治療後に発生した網膜色素上皮剝離（PED）の報告。

所見と経過：症例は50歳男性。受診1年前から全身の異常知覚と疼痛，耳鳴と聴覚障害，味覚障害があった。1か月前から左眼の霧視を自覚し，当院を受診した。両眼の汎ぶどう膜炎を認め，眼，肺，末梢神経サルコイドーシスと診断した。ステロイド薬は全身投与せず，局所投与のみで治療開始1か月後に消炎した。初診から8か月後，右眼に新たにPEDが生じた。自覚症状もなく，視力，視野とも異常を認めず，PEDの確認4か月後に消失した。

結論：眼サルコイドーシスは消炎後も脈絡膜病変の発生に注意する必要がある。

Abstract　Purpose: To report a case who developed retinal pigment epithelial detachment during the course of treatment for ocular sarcoidosis.

Case: A 50-year-old man presented with general dysesthesia, pain, tinnitus, hearing difficulty and dysgeusia since one year before. He was diagnosed with ocular, pulmonary and neuro-sarcoidosis. Ocular inflammation was cured after one month of topical treatment with steroid only. His right eye developed retinal pigment epithelial detachment 8 months later. He was free of subjective symptoms, visual acuity, or visual field. Retinal pigment epithelial detachment disappeared 4 months later.

Conclusion: This case illustrates that retinal pigment epithelial detachment may develop as asymptomatic complication of ocular sarcoidosis.
