要約　目的：急激に眼底所見が変化した糖尿病網膜症の症例の報告。症例：64歳男性が交通外傷を機に糖尿病が発見され，その翌日に眼科を受診した。矯正視力は右0.9，左0.7で，眼底に軟性白斑と，線状としみ状出血があった。血圧は136/76mmHg, HbA_1cは8.5％であった。1か月後の再診で出血斑と軟性白斑がやや増加し，2か月後にはさらに増加した。血液検査で糖尿病の状態に大きな変化はなく，初診時から赤血球267万/μl，ヘモグロビン8.5g/dl，ヘマトクリット値26.9％の貧血があったことが判明した。4か月後から貧血の回復とともに眼底出血と白斑は減少した。結論：糖尿病網膜症が急激に悪化する場合には，貧血が原因として存在する可能性がある。

Abstract.　Purpose：To report a case of diabetic retinopathy that showed radical changes during a short period. Case and Findings：A 64-year-old male suffered a traffic accident leading to the detection of diabetes mellitus. When seen the following day,his visual acuity was 0.9 right and 0.7 left. Both eyes showed soft exudates,blot and linear hemorrhages. His blood pressure was 136/76 mmHg with HbA_1c value of 8.5％. Fundus hemorrhage and exudates showed an increase 1 month later and further 2 months later. Hematological examinations showed minor change in the control of diabetes and the presence of anemia with erythrocyte cell count of 2,670,000/μl,hemoglobin level of 8.5 g/dl,and hematocrit value of 26.9％. Fundus findings improved after treatment for anemia 4 months later. Conclusion：Anemia may be present when diabetic retinopathy shows acute exacerbation without apparent cause.
