要約　目的：ぶどう膜炎で発症し，メトトレキサートの大量療法が奏効した眼・中枢神経系原発の悪性リンパ腫の症例報告。症例と経過：56歳女性が右眼の視力低下で受診した。矯正視力は右0.03で，眼底の後極部と鼻側に網膜下黄白色滲出斑があった。プレドニゾロンの内服でいったんは改善したが，網膜周辺部に出血と血管炎を伴う滲出斑が1か月後に出現した。画像診断で頭蓋内に結節性病変が多発していた。硝子体生検では陰性，脳病変の生検で悪性リンパ腫と診断された。メトトレキサートの大量全身療法と髄腔内注入を行い，眼と頭蓋内病変は改善し，最終視力0.04を得た。残存する頭蓋内病変には放射線療法を追加した。結論：本症例では眼・中枢神経系の悪性リンパ腫にメトトレキサートの大量全身療法と髄腔内注入が奏効した。

Abstract.　Purpose：To report a case of malignant lymphoma of central nervous system（CNS）which initiated as uveitis and which responded to massive dosis of methotrexate. Case and Findings：A 56-year-old female presented with decreased vision in her right eye. The corrected visual acuity was 0.03. The right eye showed subretinal exudates in the central and nasal fundus. Systemic prednisolone was effective but was followed by hemorrhage and exudates in the periphery one month later. Diagnostic imaging showed nodular lesion in the brain. Vitreous biopsy was negative. Biopsy of the brain led to the diagnosis of malignant lymphoma. Ocular and CNS lesions improved after intrathecal and systemic methotrexate in massive dosis with final visual acuity of 0.04. Conclusion：This case illustrates that ocular and CNS malignant lymphoma may respond to intrathecal and systemic methotrexate in massive dosis.
