要約　目的：難治性網膜芽細胞腫に対し，メルファランによる保存的治療法が奏効した症例の報告。症例：台湾で出生した女児が生後1年9か月で両眼の網膜芽細胞腫と診断された。両眼ともReese-Ellthworth分類のVbであった。ビンクリスチン，エトポシド，カルボプラチンの3剤併用による全身化学療法を6コース受けたが完治せず，他の3剤併用による化学療法をさらに3コース受けた。腫瘍が不活性化しないため，放射線外部照射が両眼に行われ，右眼には冷凍凝固と光凝固とが行われた。右眼に硝子体出血が発症し，生後2年10か月で当科を受診した。所見と経過：右眼に腫瘍と硝子体播種，左眼に腫瘍があった。メルファランの選択的眼動脈注入と硝子体注入を行い，さらに灌流液にメルファランを加えた硝子体手術を行った。生後3年10か月の検査で左右眼とも腫瘍の再発または転移はなく，矯正視力は右光覚弁，左0.07であった。結論：難治性網膜芽細胞腫に対し，筆者らが開発したメルファランによる保存的治療法は有効かつ安全であった。

Abstract.　Purpose：To report the outcome of local chemotherapy with melphalan in a case of bilateral retinoblastoma. Case：A female infant was diagnosed with bilateral retinoblastoma at the age of 1 year 9 months. The tumor was graded as Vb after Reese-Ellthworth in either eye. The tumor failed to respond to 6 courses of systemic vincristine, etoposide and carboplatin. The tumor persisted after 3 courses of chemotherapy with other agents. Both eyes then received external radiation. The right eye was treated by cryotherapy and photocoagulation. She was referred to us at the age of 2 years 10 months. Findings and Course：The right eye showed tumor with vitreal seeding. The left eye showed a tumor mass. Both eyes were treated by injection of melphalan into the vitreous and ophthalmic artery. Vitreous surgery was performed using perfusion fluid containing melphalan. Both eyes had no recurrence or metastasis of tumor when seen at the age of 3 years 10 months. Conclusion：Local chemotherapy with melphalan was developed by us and was safe and effective in a bilateral case of refractive retinoblastoma.
