　76歳男性が右視野障害で受診した。右眼底には非定型的脈絡膜欠損とその下方周囲に網膜剝離がみられ，裂孔は不明であった。超音波白内障手術，硝子体切除術，ガスタンポナーデを行った。術中，脈絡膜欠損部辺縁全周には肥厚した後部硝子体膜が強く付着していた。この肥厚した変性後部硝子体膜を辺縁から剝離・除去し，後部硝子体剝離を作製すると，脈絡膜欠損部の下方辺縁に接した健常部網膜に裂孔がみられた。術後，OCTでは網膜下液が存在していたが，検眼鏡的に裂孔は閉鎖し，網膜は復位した。術中に直視下に裂孔を検索でき，裂孔への牽引を解除できる硝子体手術は，脈絡膜欠損に合併した網膜剝離に有効であると思われた。

A 76-year-old male presented with impaired visual field in his right eye. The corrected visual acuity was 0.1 right and 0.9 left. The right fundus showed atypical choroidal coloboma superior temporal to the macula and retinal detachment in the inferior quadrant. We treated him by phacoemulsification-aspiration,vitrectomy,and gas tamponade. During surgery,a retinal tear was detected along the inferior margin of the coloboma. The tear was adherent to the vitreous jel. After surgery,the retina was apparently reattached. Observation by optical coherence tomography（OCT）,however,showed shallow residual retinal detachment which gradually became reabsorbed. This case illustrates that release of vitreoretinal traction may be effective for retinal detachment in choroidal coloboma.
