62歳女性が前日午後からの視力低下で受診した。8年前にC型肝炎ウイルスが陽性でインターフェロンの投与を6か月間受けている。矯正視力は右0.01，左0.02であり，軽度の視神経乳頭浮腫が両眼にある以外には顕著な異常はなかった。メチルプレドニゾロンによるパルス療法を1週間間隔で3クール行ったが視力は改善しなかった。発症から4週後に下肢にしびれを訴えるようになり，頭部MRI検査で脱髄巣が発見され，視神経炎で初発した多発性硬化症と診断された。このような非可逆的で激しい視力低下を伴う多発性硬化症は稀有な病型であり，調べた限りではいままで報告されていない。

A 62-year-old female presented with bilateral visual loss since previous afternoon. She had received interferon therapy for hepatitis C virus 8 years before. Her corrected visual acuity was 0.01 right and 0.02 left. Mild optic disc edema in both eyes was the sole abnormal finding. Three courses of pulsed corticosteroid therapy failed to induce improved visual acuity. She complained of numbness in the lower extremity 4 weeks after onset of eye problems. Magnetic resonance imaging（MRI）of the brain showed a demyelinating lesion,leading to the diagnosis of multiple sclerosis with acute optic neuritis as the initial manifestation. This type of multiple sclerosis appears to be extremely rare. To our best knowledge,there has been no report describing a similar case.
