大量の線維素析出を伴うぶどう膜炎を発症した潰瘍性大腸炎の1例

Uveitis with massive fibrinous exudation in a case of ulcerative colitis

29歳女性が左眼の充血と疼痛で受診した。2年前に潰瘍性大腸炎と診断され，副腎皮質ステロイド薬による治療を受けたことがある。矯正視力は右眼1.2，左眼光覚弁であった。右眼には病的所見はなく，左眼には毛様充血，前房に強い線維素滲出，虹彩後癒着があり，眼底は透見できなかった。副腎皮質ステロイド薬点眼などで，前部ぶどう膜炎は急速に軽快し，左眼視力は翌日0.3，1か月後に1.0になった。眼底と蛍光眼底造影所見は正常であった。初診から1年後の現在まで眼病変の再発はない。潰瘍性大腸炎に伴うぶどう膜炎は一般に軽症であるとされているが，劇症型もあることを本症例は示している。

A 29-year-old woman presented with hyperemia and pain in her left eye. She had been diagnosed with ulcerative colitis two years before. It subsided following systemic corticosteroid. Her corrected visual acuity was 1.2 right and light perception left. Her right eye showed normal findings. Her left eye showed ciliary injection,posterior synechia,and massive fibrinous exudation in the anterior chamber. The anterior uveitis rapidly subsided after treatment with topical corticosteroid and mydriatics. The visual acuity improved to 0.3 on day 2 of treatment and to 1.0 one month later. The left fundus was normal when examined by funduscopy and fluorescein angiography. There has been no recurrence for one year until present. This case illustrates that,contrary to the accepted view,uveitis may run a fulminant course when associated with ulcerative colitis.
