 A 60-year-old woman presented with decreased visual acuity in her right eye. She had been diagnosed with iritis 15months before. Her visual acuity was 0.15 right and 1.0 left. Her right eye showed iritis and vitreous opacity. An exudative mass was present in the peripheral retina. Fluorescein angiography showed no extravasation from retinal vessels. Vitreous opacity increased during the ensuing 10months. Detailed examinations were inconclusive but suggestive of masquerade syndrome as a possibility. Systemic prednisolone was followed by marked improvement of ocular lesions. Sudden onset of purpura,pancytopenia and anuria led to the detection of multiple cerebral infarction. Brain biopsy led to the diagnosis of intravascular lymphoma. The patient passed away 10months later. This case illustrates that masquerade syndrome may respond to systemic corticosteroid.
