　小児の脊柱は，柔軟で弾力性に富み，生理的可動域が大きい．このため脊椎損傷は成人のそれと比較して少ない．今回，1歳女児の軸椎歯突起骨折の1例を経験して，4年間の経過観察を行った．症例は，1歳4カ月の女児である．自動車の後部座席のチャイルドシートに着座中に，父親の居眠り運転が原因で，水田に転落して受傷した．軸椎歯突起骨折の診断で，当科で入院治療を行った．頚椎伸展位で骨折が整復されることが，単純X線写真で確認された．このため頚椎カラーによる固定と床上安静による保存療法を行った．受傷後10週の単純X線写真の機能撮影で骨癒合が確認された．受傷後4年の時点で特に愁訴なく，日常生活を送っている．小児の歯突起骨折は，愁訴が不明瞭で，かつ神経学的所見が把握しにくい．このため初診時の診断がときに困難である．小児で頚部の姿勢が不自然な場合は，頚椎の損傷を疑う必要がある．さらに，単純X線写真の軟部組織陰影の増大により骨傷の存在を疑うことができる．

　We reported a case of odontid process fracture in one year old child with follow-up of 4 years.　The patient was one year and 4 months old girl.　She was injured as the result of motor vehicle accident.　Odontoid process fracture was reducted by extention position of cervical spine.　Therefore, she was treated conservatively with neck brace immobilization and bed rest.　Ten weeks after the accident, bony uion of the fracture was comfirmed in plain X-ray film.　Four years after the injury, she had no neck complaint.　Diagnosis of odontoid process fracture in children is difficult because of unclear complaint and neurologic findings.　If cervical spine is unnatural position in children, we should suspect cervical spine injury.
