　セメントレス大腿骨人工骨頭置換術における骨温存型ステムの有用性を検討した．1996年1月から2005年12月までに，人工骨頭置換術を施行した85例を対象とし，臨床評価としてJOAスコア，大腿部痛および合併症を，X線学的評価としてステム側とカップ側を評価した．最終経過観察時平均JOAスコアは74点，大腿部痛発生は5例であった．合併症は脱臼1例と術中大腿骨骨折各1例であった．全例bone-ingrown fixationが得られ，ステムの沈下は1例にみられた．Stress shieldingは19例(22.4％)に認められた．良好な術後成績を示したと考える．今後の経過観察が重要である．

　We report the middle-term results of a clinical and radiographic evaluation of 85 hips of cementless hemiarthroplasty (with Bone-preservation type stem).　The average follow-up period was 4 years 1 month.　The average JOA score at the last follow-up period was 74.　Five patients (5.9%) had thigh pain.　There was one dislocation and one intra-operative fracture of femoral shaft.　All cases obtained bone-ingrown fixation.　There was one sinking of stem.　Mild stress shielding was observed in 19 cases (22.4%).　It is important to follow the patient's course continuously.
