 We report an unusual case of traumatic spondylolisthesis of the axis. A 50-year-old-man fell down the stairs and suffered a severe contusion of the occiput. He complained of severe neck pain but showed no evidence of a neurological deficit. Cervical spine x-rays and computed tomography (CT) revealed a horizontal fracture line extending from the lamina to the spinous process of C2 and left C2/3 facet dislocation. The dislocation was reduced by Crutchfield skull tong traction, but it was difficult to maintain the anatomical alignment with a SOMI brace, and C2-3 posterior fusion was performed. The patient had no neck pain and no neurological deficits at the follow-up examination 1 year after surgery.
