　会話時においてのみ口部に振戦が出現した66歳の女性を経験した。既往歴，家族歴，検査所見はすべて正常であった。表面筋電図では会話時においてのみ口輪筋に4-5Hzの規則正しい群化放電の出現が認められた。単純な発声や口笛，嚥下や咀嚼時には振戦の出現は認められなかった。したがって，本例の振戦は会話時においてのみ出現する課題特異的振戦（task-specific tremor）であると考えられた。少量のアルコールで抑制され，かつβ遮断薬も有効であったことから，本態性振戦の亜型として位置づけられる可能性も考えられた．課題特異的異常運動は書痙に代表されるように上肢において認められる場合が通常であり，本例のように会話時に特異的に出現する口部の振戦はきわめて稀である。

We report a 66-year-old Japanese woman in whom tremor of lips appeared during speech. Her past and family histories were unremarkable. On neurological examination, there was no abnormal finding except the lip tremor. Results of laboratory findings were all within normal levels. Her MRI and EEG were normal. Surface EMG studies revealed that regular grouped discharges at a frequency of about 4-5 Hz appeared in the orbicularis oris muscle only during voluntary speaking.
