　症例は71歳女性。糖尿病にて食事療法中。左慢性硬膜下血腫洗浄術1カ月後熱発，全身倦怠感が出現し来院。白血球10,700／mm³，CRP 11.50mg／dl，尿中白血球50／F，細菌3＋であり，尿路感染症を疑い抗生物質を投与したが，右不全麻痺（−5／5）が出現した。頭部CTにて硬膜下血腫の増大を認め，穿頭術を施行したところ黄白色に肥厚した被膜を認め，硬膜下血腫内膿瘍と考えドレナージを行った。尿および膿瘍からはKlebsiella pneumoniaeが検出された。頭部CT上，膿瘍腔はいったん縮小したが再増大したため被膜摘出術を行い，症状は改善した。高齢者や，免疫能が低下し感染症を有する患者に慢性硬膜下血腫が合併した場合，穿頭術後にこのような病態が生じる可能性があり，適切な対応を念頭に置いて治療にあたることが勧められる。

A 71-year-old female complicating Parkinson dis-ease and diabetes mellitus was admitted to our medi-cal center for urinary tract infection one month after burr hole irrigation and drainage of left chronic sub-dural hematoma. Klebsiella pneumonia was detected in the bacterial culture of her urine. As antibiotic therapy started, her condition and peripheral white blood cell counts were improved. But consciousness level got worsened and right hemiparesis appeared. A CT scan showed re-accumulation of left subdural fluid so an emergent irrigation was performed.
