Abstract 文献概要

　症例は典型的な三叉神経痛の65歳女性。通常のCT，MRIでは小脳橋角部に異常がみられず，特発性三叉神経痛として25年以上ペインクリニックで治療を受けていた。MRIでのConstructive interference in steady state（CISS）法および拡散強調画像を行ったところ右小脳橋角部に病変が描出され，類上皮腫による症候性三叉神経痛と、診断し腫瘍摘出術を行い三叉神経痛は消失した。三叉神経痛のMRI検査では，小脳橋角部脳槽内に存在する小さな類上皮腫を鑑別するために，拡散強調画像もあわせて行う必要がある。

A 65-year-old woman had been conservatively treated as idiopathic trigeminal neuralgia for over 25 years, because conventional computed tomography (CT) and magnetic resonance imaging (MRI) showed no abnormality in the cerebello-pontine (CP) angle cistern. She received a detailed MRI by constructive interference in steady state and diffusion weighted im-age (DWI) sequences. Those sequences on MRI well demonstrated a epidermoid tumor in the CP angle cis-tern, and the removal of the tumor completely re-solved the neuralgia.
