　脳死に至る過程における脳温と脳圧について，虚血側と非虚血側を同時にかつ持続的に測定できた急性内頸動脈閉塞症例を経験した。症例は73歳男性。意識障害を主訴に当科へ搬送された。右半身の完全麻痺と失語症を認め，画像所見で左内頸動脈閉塞による脳梗塞（脳血栓症）と診断した。両側中大脳動脈領域に脳温センサーと脳圧センサーを脳実質内に挿入し，脳温と脳圧を持続的にモニタリングした。挿入時には梗塞巣の脳温は低く，脳圧は高く左右差を認めた。脳圧は40 mmHgを越えた時点で左右とも同じとなり，さらに上昇し脳ヘルニアとなった。脳温は脳ヘルニアとなった後，左右とも膀胱温より低くなった。本例では，虚血状態が進行し，脳血流が途絶し，脳組織温が低下したというプロセスがリアルタイムに観察できた。

We experienced a patient with acute thrombosis of the unilateral internal carotid artery. We monitored the brain tissue temperature and intracranial pressure not only in both hemispheres simultaneously but also continuously throughout the process of brain death.

The patient was a 73-year-old male who presented to our emergency room with right hemiparesis and aphasia. On admission to our department, no specific pathological findings were identified by brain CT. However a following investigation with left carotid ar-teriogram demonstrated a complete occlusion of the left internal carotid artery.
