I．はじめに

　内頸動脈閉塞および中大脳動脈閉塞例に対する浅側頭動脈‐中大脳動脈（superficial temporal artery-middle cerebral artery, STA-MCA）吻合術は，1985年のEC-IC bypass study^17）で脳虚血によるstrokeを防止できなかったと報告され，われわれ脳神経外科医にとって不本意な結果であった．しかし，EC-IC bypass studyではbypass手術を行った群で周術期にmajor strokeを4.5％も起こしていたことがその後のbypass群の予後評価に重大な影響を及ぼし，strokeの予防に内科治療群と比較して有意な差が出なかったと考えられる．さらに，脳血流の評価による患者の選択がされていなかったために，hemodymamic comprom—iseにある患者以外，たとえば脳塞栓による脳虚血例にも手術がされていた可能性があり，そのために，有意な差がでなかった可能性も考えられている^6,15）．

　著者らは，1988年からは内頸動脈閉塞および，中大脳動脈閉塞例に対し，脳血流検査の評価を参考にしてSTA-MCA吻合術を行ってきた．

[Purpose]We studied cerebral circulation in patients with occlusion of the main cerebral artery and in-vestigated the efficacy of STA-MCA anastomosis.

[Patients and methods]Thirty-six patients with occlusion of the main cerebral artery were studied.Twenty-three patients had occlusion of the internal carotid artery and 13 had occlusion of the middle cere-bral artery. The mean age was 62 years. Cerebral blood flow (CBF) was measured in all patients and cere-brovascular reactivity (CVR) was examined in 11 patients by xenon enhanced CT. Intraoperatively, corti-cal arterial pressure and anastomotic flow were measured.
