Abstract 文献概要

　慢性期の一側内頸動脈閉塞症の症例にmulti-voxelの¹H-MRSを行い，脳血流量との関係について検討した。対象は15例，全例Rankin Disability Scale 1から3までで，画像上も粗大な梗塞巣は認めなかった。¹H-MRSの測定はSE−2 D-CSI法，脳血流量はXenon-CT法により側脳室上端直上で行った。患側皮質のNAA／Crは8例で低下していた。患側皮質血流量が低下した3例では，全例で皮質NAA／Crが低下していた。患側皮質血流量が正常の12例中，7例では皮質NAA／Crは正常であった。患側皮質NAA／Crと皮質血流量がともに正常下限域にある症例では，NAA／Crとダイアモックス負荷による脳血流量増加率には弱い相関関係が示唆された。慢性期の一側内頸動脈閉塞症のmulti-voxelの¹H-MRSによるNAA／Crの測定は，脳代謝の把握に臨床的に有用である。

Proton magnetic resonance spectroscopy (¹H-MRS) has less been used to analyze cerebral metabolism in ischemic lesions compared to single photon emission computed tomography or positron emission computed tomography. Recent advances in magnetic resonance imaging apparatus and the related software have made possible obtaining multi-voxel ¹H-MRS in a single study. We examined multi-voxel ¹H-MRS in patients with unilateral internal carotid artery (ICA) occlusion to study the relationship between cerebral metabolism and cerebral blood flow.
