生検により診断された脳幹脳炎の1例

A Case of Brainstem Encephalitis Diagnosed by Stereotactic Biopsy

Ⅰ．はじめに

　ウイルス性脳炎は，治療開始時期が後遺症状，予後に重大な影響を及ぼすため，より早期から開始することが重要とされている．しかし病原ウイルスの特定率は必ずしも高くなく，またその結果に時間を要することから，非典型的な臨床経過を示したり，画像所見が腫瘍性疾患や血管障害と類似するような症例では診断自体が困難となる．

　脳炎を強く疑ったが，主病巣が稀な部位に存在し，さらに悪性リンパ腫との鑑別が難しいため臨床所見のみでは診断に至らず，確定に生検を必要としたウイルス性脳幹脳炎の1例を経験したので報告する．

Brainstem encephalitis is a rare form of encephalitis which should be differentiated from cerebrovascular and neo-plastic diseases. The authors report a case of viral brainstem encephalitis mimicking malignant lymphoma. A 55-year-old female was admitted to our hospital with gradually progressive diplopia and left hemiplegia. CT scan revealed low density lesions in the right globus pallidus and the anterior limb of the internal capsule. MRI demon-strated high intensity signals extending into the right cerebral peduncle, temporal lobe, thalamus and the contralat-eral thalamus on FLAIR images.
