　突然の後頭部痛で発症した特発性頭蓋内圧低下症の44歳・女性例を報告した。頭痛は坐位・立位で増悪し，臥位で軽減する特徴があり，髄液検査では初圧：55mmH₂0，細胞数：38／mm³，蛋白：57mg／dlであった。頭部MRIではT1強調画像でガドリニウム投与により脳表に沿った部分がびまん性に造影され，また，subdural fｌuid collectionsを認めた。髄膜生検では硬膜の外側面およびくも膜には異常を認めなかったが，硬膜内側面のdural border cell layerに非特異的肉芽組織が認められ，これがMRIの造影効果を示した部位に一致すると考えられた。この非特異的肉芽組織は頭蓋内圧の低下により脳が下方に偏倚し，それに伴って生じた二次的な変化と考えられた。

We report a 44-year-old woman with spontane-ous intracranial hypotension (SIH) who developedan acute, severe, nonthrobbing headache. The head-ache remained more severe in the occipital region and was markedly worse with upright posture. Cerebrospinal fluid (CSF) examination revealed an opening pressure of 55 mmH₂O, and the CSF contained 38 cells/mm³ and 57 mg/dl of protein. The results of other laboratory examinations were unremarkable. T1 weighted MR images (MRI) of the head revealed an extensive diffuse pachymenin-geal gadolinium enhancement and bilateral subdur-al fluid accumulation.
