　内頸動脈形成不全に前交通動脈瘤，左右内頸動脈間吻合，左側rete carotidisを合併した極めて稀な1例を経験したので報告する。症例は49歳，男性。くも膜下出血で発症。頭部CTにて左側頸動脈管の形成不全を認めた。右頸動脈造影で右A1—A2分岐部に前交通動脈瘤，海綿静脈洞部に頸動脈間吻合血管，左A1形成不全を認めた。左頸動脈造影では左内頸動脈形成不全と上顎動脈，上行咽頭動脈などから左内頸動脈C3-5部に環流するtortuousな吻合血管を認めた。この吻合血管はHawkinsらの報告した"rete carotidis"であり，内頸動脈欠損や形成不全症などで稀に報告されている。頸動脈間吻合も一側の頸動脈欠損，形成不全症などで稀に報告がみられる。本症例の発生機序として，左側内頸動脈形成不全により左側外頸動脈系と右側内頸動脈から吻合血管が発達し，右側からの血流によるhemodynamic stressなどが加わり前交通動脈瘤が発生したものと推察した。

Hypoplasia of the internal carotid artery with cerebral aneurysm is extremely rare. The authorsreport a case of hypoplasia of the left internal carotid artery associated with anterior communicat- ing aneurysm, intercarotid anastomosis and left rete carotidis.

A 45-year-old man was admitted to Minamata Municipal Hospital because of severe headache and disturbance of consciousness. CT scan showed subarachnoid hemorrhage with intracerebral hematoma. Right carotid angiogram showed the anterior communicating aneurysm arising from the right A1-A2 corner and intercarotid anastomosis on the cavernous portion.
