Abstract 文献概要

　64歳時，心気症状などの精神症状で発症し，精神症状および人格変化を主体に経過し，約6年の経過で外傷性硬膜下血腫にて死亡した石灰沈着を伴うびまん性神経原線維変化病（diffuse neurofibrillary tan—gles with calcification：DNTC）の1剖検例を報告した。本例は，大脳全体に及ぶ多数の神経原線維変化の出現と老人斑の欠如，およびFahr病様の広汎な石灰沈着が認められたが，限局性萎縮を欠く点で従来の報告と異なっていた。この理由は，広汎な硬膜下出血による脳実質の浮腫性変化の影響，あるいは病期が比較的早期であるため萎縮が進行しなかった可能性が考えられた。本例は，大脳白質を主とした小細動脈の硝子様変化および基底核部を中心とした多発性の脳梗塞が認められた点が特徴的であった。本例をDNTCに高血圧性変化が合併した症例と捉えることもできるが，血管変化がDNTCの本質的変化である可能性を指摘した。

"Diffuse neurofibrillary tangles with calcification (DNTC)" is a slowly progressive form of presenile dementia characterized by localized temporal atro-phy, pronounced calcareous deposits and numerous neurofibrillary tangles (NFTs) without senile pla-ques. We report a 70-year-old woman with DNTC, multiple infactions and hyaline arteriosclerosis. This case was clinically characterized by persistent delusional ideas and personality changes. Intel-lectual deterioration was mild, and no focal mani-festations were noted.
