皮質下出血を呈したisolated angiitis of the CNSの1例—benign typeについて

Isolated Angiitis of the Central Nervous System Presenting as Subcortical Hemorrhage:A Case Report of Benign Type

　症例は47歳女性で，頭痛が先行症状として存在した。トイレで怒責中に右下肢の脱力としびれ感が出現し，搬送された。搬送時，神経学的には意識清明で軽度の右片麻痺を呈し，CT上，左頭頂部に皮質下出血を認めた。検査所見上，全身の炎症所見は認められず，抗核抗体や凝固機能にも異常を認めなかった。発症から3日後の脳血管撮影では左前大脳動脈中枢側にソーセージ様の拡張が存在し，他にも複数の血管にseg—mentalな壁不整像が認められた。中枢神経系の孤発性血管炎と診断し，副腎皮質ステロイドを30mg／日から投与を開始し，漸増予定であったが，2カ月後に脳血管撮影を再検し，一部，血管壁不整像が遺残するも，著明な改善が得られたため，徐々に副腎皮質ステロイドを減量した。その後，神経症候の増悪はみられていない。本疾患には予後の良好なタイプも存在することが確認され，早期診断，治療が望まれた。

A 47-year-old woman had an episode of severe headache for a few days. She suddenly experienced right leg weakness and sensory loss. A CT scan revealed subcortical hematoma at the left parietal lobe on admission. Cerebral angiography showed multiple vascular irregularities such as segmentally narrow or sausage like dilatated areas. Her labora-tory studies were entirely normal including anti-nuclear antibody and coagulation tests. The diagno-sis of isolated angiitis of the CNS was made and she responded well to the low dose corticosteroid the-rapy.
