神経皮膚症候群

NEUROCUTONEOUS SYNDROMES

I．はじめに

　古くはdysplasies neuro-ectodermiques congenitales^1）というよび方もあつた。この病名の背景には皮膚（表皮および付属器上皮）と神経とが外胚葉を共通起源とする考え方がある。しかしながら，胎生期表皮基底細胞，神経管，眼球網膜，神経櫛（neural crest）の分化する以前に，神経皮膚奇形および奇形成疾患（neurocutane Miβbildungen und Miβbildungskrankheiten）の規定（Determinierung）が果して起こりうるであろうか？　Bourneville-Pringle母斑症で複数胚葉にわたる発生異常が考えられたこともあつたが，それにしては末梢神経に見るべき病変のないことが不思議である。神経皮膚症候群は，近年にいたるまで，ほとんど奇形性皮膚疾患に限られたかの感もあつたが，今日ではその他の神経皮膚症候群に関する知見が豊富に蓄積され，その遺伝機構や発病機序もかなり詳かになつている。神経皮膚症候群としてRayrnond D. AdamsおよびWilliam B. Reed^2）（1971）は80余種の病名を挙げ，R. J. Cairns^3）（1972, Table 62.12）はDisorders of skin and nervous systernとして50余種の病名をあげている。このように多数の疾患について，総説することは限られた紙面で難かしい。それよりも，あまり数多くない疾患につき，ある程度系統的に述べた方が意義があるかと思われる。皮膚症状は多くの皮膚科書に記されているし，神経の病変については読者諸賢の方が著者よりも詳しく御承知のことと思う。したがつて，神経病変と皮膚病変との背後にあつて両者を併発せしめる機序になるたけ焦点を絞つて記したい。個々の疾患に就いての記載をできるだけ簡単にしたから，詳細を調べられる場合の便宜を考えて文献を挙げる労をいとわなかつたつもりでいる。

　神経皮膚症候群には遺伝性疾患が多いが，遺伝に関する記載は西田尚史著皮膚病の遺伝^4）に拠つた。

　The border-line is rather arbiturary between neurocutaneous syndromes and other skin disorders with posibility of being accompaniced with nerve symptoms. More than eighty disorders were classified by Adams and Reed (1971) as neurocutaneous syndromes, while Carns (1972) mentioned approximately fifty neurocutaneous syndromes. In phakomatoses the simultaneous occurence of skin and nerve lesions may be explained by the distribution of phakomataklasts (T. Kawamura u. a. : Hautarzt. 15: 476, 1964) in the both organs. Phakomatoklasts may originate not only in the ectoderm (neural crest) but also in the mesenchyma. The significance of the rôle played by the blood vessels and connective tissue was stressed in phakomatoses and other neurocutaneous syndromes. Numerous neurocutaneous syndromes have been suggested to be caused by kiochemical defects ane malnutrition. In the group of neurocutaneous syndromes where nerve lesions are accompanied with epidermal (epithelial) lesions, the reason why both lesions should necessarily occur simultaneously is obscure. Among acquired neurocutaneous syndromes, Behçet disease, systemic lupus erythematosus and nerve disorder caused by metastasis of or presumable immunological response to skin malignancies are important.
